Before Your Appointment

  • Complete your online forms.
  • Bring your insurance card and photo ID.
  • Arrive 10–15 minutes early if your forms are incomplete.

What to Expect

  • Comprehensive exam
  • Digital X-rays, if needed
  • Professional cleaning, when appropriate
  • Personalized treatment recommendations

Bring With You

  • A current medication list, if applicable
  • Recent dental X-rays, if available

Patients Under 18

A parent or legal guardian must accompany patients under the age of 18.
